WREB receives numerous requests for scores to be sent to various state dental boards. All state dental boards require different items.
Prior to sending your request for scores, please be sure to contact the state board in which you wish to practice, to better determine what you will need.
Some of the WREB member states require the same item in different forms; WREB has compiled a listing to see if you will need to use our services.

WREB will send this once you have completed and passed your exam. Original success cards will only be sent out one (1) time. Duplicates may be requested. |

In order to help us provide the appropriate documentation, please assist us by specifying what is needed. WREB automatically sends reports on all candidates to our member state boards; however, you are ALSO required to send proof of your exam score(s). If you are applying for licensure in a non-member state, you will need to use our services. Please use a separate form for each request and return with appropriate fees.

What it is: Copy of the following:
This information does NOT include scores.
Detailed Scores
What it is: This letter provides the breakdown of scores received in each section of your Dental or Dental/Hygiene exam, with a WREB raised seal.
Success Card
What it is: For those candidates who originally received a certificate, this is a replica of the success card we presently use. It shows only the total score you received; it does not provide a score break down.
Certificate of Passing
What it is: This 8½" x 5½" certificate is suitable for framing. It states your name, location, month and year that the exam was taken and passed; no score is documented.
Candidate Performance Review
What it is: This document breaks down the score received on each section of the exam. Unsuccessful candidates automatically receive this review with their result letters.
Dental Hygiene Summary Profile Sheet
What it is: This is a summary sheet for unsuccessful candidates only which charts each validated tissue trauma, calculus, and probe and recession error.
Dental Examination History
What it is: For candidates (currently California only) who are only required to show proof of exam completion and status of pass or fail. This information is sent directly to the California State Board. Download this form here.
